Roles & Responsibilities

• Assess patients' functional abilities and develop individualized outpatient treatment plans
• Implement evidence-based therapeutic interventions to maximize independence and daily living skills
• Document evaluation, treatment, and progress clearly and timely in Cerner
• Collaborate with physicians, nurses, and interdisciplinary team members to coordinate patient care
• Educate patients and caregivers on home programs, adaptive equipment, and safety strategies
• Monitor outcomes and modify treatment plans to ensure measurable progress toward goals
• Participate in unit meetings and contribute to continuous improvement efforts in the outpatient clinic

Qualifications

• Active Occupational Therapist licensure or eligibility to obtain licensure in the state of California (able to submit)
• BLS (Basic Life Support) certification required
• Minimum 2 years of Occupational Therapy experience (outpatient and/or acute care experience a plus)
• Experience with Cerner or similar electronic health record/charting systems
• Strong communication and documentation skills and ability to work as part of an interdisciplinary team
• Current professional liability coverage as required by assignment/policy

Education

• Master’s or Doctorate in Occupational Therapy or equivalent professional degree preferred

Knowledge & Skills

• Skilled in outpatient OT evaluation and treatment planning
• Knowledge of acute care transitions and rehab principles
• Proficient with Cerner charting and electronic documentation workflows
• Ability to perform functional assessments, ADL training, and adaptive equipment recommendations
• Patient education, family training, and discharge planning expertise
• Comfortable working in clinic/outpatient and allied health settings
