Roles & Responsibilities

• Provide comprehensive, patient-centered evaluations and create individualized treatment plans that restore function and independence
• Deliver hands-on manual therapy, therapeutic exercise and evidence-based interventions to help patients reach real-world goals
• Educate patients and families in home programs and injury prevention to maximize long-term outcomes
• Collaborate with clinic staff and interdisciplinary partners to coordinate care and track progress
• Accurately document assessments, treatment plans and progress notes in the clinic EMR
• Contribute to a positive clinic culture by mentoring colleagues and supporting community outreach

Qualifications

• Active Minnesota Physical Therapist license or eligibility to obtain licensure in MN
• Doctor of Physical Therapy (DPT) preferred; Master's in Physical Therapy considered
• Current BLS/CPR certification
• Outpatient physical therapy experience preferred; travel therapy experience is a plus
• Able to complete onboarding with at least 30 days notice
• Reliable transportation and ability to commute to clinic sites as needed

Education

• Doctor of Physical Therapy (DPT) preferred; Master's in Physical Therapy accepted

Knowledge & Skills

• Skilled in outpatient musculoskeletal evaluation and treatment
• Proficiency with manual therapy and therapeutic exercise progression
• Strong patient education and motivational coaching skills
• Familiarity with EMR documentation and outcomes tracking
• Good time management, clinical reasoning and teamwork abilities
