Shift Type: Night shift, 7:00 PM - 7:00 AM NOC; Friday and Saturday nights every third weekend; Sundays are part of the regular work week with some flexibility

Roles & Responsibilities

• Assess patients to determine mental, physical and spiritual needs and tailor care plans
• Develop, implement and evaluate individualized plans of care in collaboration with the multidisciplinary team
• Use clinical judgment to recognize and report changes in patient condition promptly
• Maintain complete and accurate medical record documentation for quality and continuity of care
• Provide clear explanations and education to patients and responsible parties to support recovery
• Contribute to a healthy workplace by managing conflict professionally and communicating effectively
• Meet Core Measure responsibilities and participate in continuing education to stay current

Qualifications

• Current Illinois Registered Nurse license (required)
• Current CPR/BLS certification (required)
• Experience in inpatient rehabilitation preferred
• Stroke certification is a strong plus but not required
• Comfort working nights and participating in a rotating weekend schedule
• Strong clinical judgment, documentation skills and ability to collaborate with a multidisciplinary team

• Patient and caregiver education with empathy and clarity
• Familiarity with neuro and stroke care is helpful
