Roles & Responsibilities

• Assess and evaluate rehabilitation needs of senior residents to create individualized treatment plans
• Deliver skilled physical therapy interventions to improve mobility, strength, and function
• Provide compassionate care that promotes independence and quality of life for older adults
• Document clinical progress and treatment plans in accordance with SNF standards
• Collaborate with nurses, OT, SLP, and interdisciplinary team members to coordinate patient care
• Educate patients and families on home programs, fall prevention, and safe mobility strategies
• Advocate for patient needs and contribute to a positive, outcomes-driven rehab environment

Qualifications

• Active CA PT License (required)
• Experience in skilled nursing facility or geriatrics preferred
• Strong clinical reasoning and patient-centered care approach
• Effective communication skills with residents, families, and team members

Knowledge & Skills

• Skilled in geriatric rehabilitation and SNF best practices
• Proficient in patient assessment, therapeutic exercise, and mobility training
• Knowledge of documentation standards and regulatory requirements in long-term care
• Familiarity with fall prevention strategies, assistive devices, and safe transfer techniques
• Team-oriented with solid interdisciplinary collaboration skills
