Roles & Responsibilities
- Assess resident mobility and functional status, then design and implement targeted therapy plans
- Deliver therapeutic exercises and gait training to improve independence and quality of life
- Monitor resident progress, adapt interventions, and communicate outcomes to the interdisciplinary team
- Document care accurately and timely in compliance with SNF and regulatory requirements
- Educate residents and families on safe mobility strategies and home exercise programs
- Collaborate with nurses and other clinicians to ensure coordinated, person-centered care
Qualifications
- Active California PTA license required
- Experience in a Skilled Nursing Facility preferred
- Strong clinical judgment and excellent communication skills with residents and team members
- Comfortable with documentation and basic SNF regulatory expectations
- Able to work full-time for the 13-week contract period
Education
- Associate degree from an accredited Physical Therapist Assistant program or equivalent certification
Knowledge & Skills
- Hands-on experience with therapeutic exercise, mobility training, transfers, and assistive devices
- Skilled in resident assessment and writing/update of treatment notes
- Familiarity with SNF workflows and interdisciplinary team communication
- Good clinical reasoning, patient safety awareness, and strong interpersonal skills
- Basic knowledge of Medicare/Medicaid considerations in post-acute care settings
