Roles & Responsibilities
- Provide skilled physical therapy evaluation and evidence-based interventions to SNF residents
- Create, implement, and update individualized treatment plans that promote function and mobility
- Document assessments, progress notes, and plans accurately and timely in the medical record
- Collaborate with interdisciplinary teams to coordinate care and achieve patient goals
- Educate patients and families on home programs, safety, and strategies to maintain gains
- Monitor outcomes, adjust interventions as needed, and ensure regulatory compliance
Qualifications
- Active California Physical Therapy license (required)
- Experience delivering skilled therapy interventions in a post-acute or SNF setting preferred
- Strong documentation skills with attention to clinical detail
- Comfortable managing caseloads and coordinating with nursing and rehab staff
- Excellent communication skills and a patient-centered approach
- Ability to work full-time for the 13-week contract period
Education
- Doctor of Physical Therapy (DPT) preferred or equivalent degree in Physical Therapy
Knowledge & Skills
- Skilled assessment and treatment planning for older adults
- Gait training, transfers, balance and functional mobility interventions
- Understanding of SNF documentation requirements and regulatory standards
- Familiarity with common post-acute diagnoses and comorbidity management
- Proficiency with electronic medical records and timely charting
- Strong clinical reasoning and outcome tracking
- Team collaboration and discharge planning
- Safe manual handling and assistive device training
