Roles & Responsibilities
- Provide skilled physical therapy interventions that improve patient function and quality of life
- Assess patients and develop individualized, evidence-based treatment plans
- Document patient status, progress, and discharge planning clearly and timely
- Collaborate with nursing and rehab team members to coordinate patient care
- Educate patients and families on safe mobility, home programs, and fall prevention
- Support facility goals for patient outcomes, compliance, and patient satisfaction
Qualifications
- Active CA PT License required
- Skilled therapy intervention and strong documentation skills required
- SNF experience strongly preferred
- Available for a full-time, 13-week contract assignment in Belmont, CA
- Professional communication and teamwork skills to work with interdisciplinary staff
Knowledge & Skills
- Strong knowledge of rehabilitation principles and PT interventions
- Skilled in clinical documentation and progress reporting
- Experience delivering care in a skilled nursing setting
- Ability to perform assessments, create treatment plans, and measure outcomes
