Roles & Responsibilities

• Evaluate residents and develop functional, person-centered treatment plans that promote independence
• Deliver targeted therapeutic interventions to improve mobility, safety, and daily living skills
• Maintain accurate, timely clinical documentation to support quality care and compliance
• Participate in care conferences and collaborate closely with the interdisciplinary team
• Ensure clinical compliance and practice ethically to protect patient well-being and facility standards

Qualifications

• Valid California Occupational Therapy license required
• SNF experience preferred and highly valued
• Strong communication skills and problem-solving abilities to support positive outcomes
• Proven ability to work effectively in a team-centered rehabilitation environment
• Available for immediate or near-term start

Knowledge & Skills

• Skilled in occupational therapy evaluation and patient-centered treatment planning
• Proficient with clinical documentation and maintaining timely records
• Comfortable working within interdisciplinary rehab teams
• Strong verbal and written communication
• Good clinical judgment and problem-solving abilities
• Teamwork and collaboration in a fast-paced SNF setting
