Roles & Responsibilities

• Evaluate residents and develop individualized treatment plans that drive functional gains
• Deliver evidence-based therapeutic interventions to improve independence and quality of life
• Complete accurate and timely documentation to support continuity of care
• Collaborate with interdisciplinary teams to coordinate holistic rehab services
• Maintain compliance with state and facility guidelines to ensure safe, high-quality care
• Advocate for residents and coach families and staff to support optimal outcomes

Qualifications

• Active California Occupational Therapy license required
• SNF experience preferred
• Strong assessment, treatment planning, and documentation skills
• Ability to work independently and as part of a rehab team
• Available to start quickly or with short notice

Knowledge & Skills

• Patient evaluation and clinical assessment
• Treatment planning and goal setting for skilled nursing residents
• Therapeutic interventions focused on functional rehabilitation
• Accurate documentation and timely charting
• Interdisciplinary collaboration and clear communication
• Familiarity with SNF regulations and compliance expectations
