Roles & Responsibilities
- Perform comprehensive screenings and evaluations to identify resident needs and create individualized treatment plans
- Deliver direct therapy that improves communication, swallowing, and overall quality of life
- Maintain complete, timely, and compliant clinical documentation to support continuity of care
- Coordinate care with an interdisciplinary team to ensure cohesive, resident-centered treatment
- Ensure compliance with facility, state, and federal standards to uphold best practices and resident safety
Qualifications
- Active California SLP License required
- Previous skilled nursing facility (SNF) experience preferred
- Strong interpersonal skills and the ability to integrate smoothly into a rehab team
- Excellent clinical documentation skills
- Available for immediate or upcoming start
Knowledge & Skills
- Speech-language pathology evaluation and treatment planning
- Clinical documentation best practices
- Interdisciplinary care coordination in a rehab or SNF setting
- Direct therapy delivery for adult/geriatric populations
- Strong communication and team collaboration skills
