Roles & Responsibilities

• Perform detailed evaluations and develop individualized, evidence-based treatment plans
• Deliver one-on-one manual therapy, balance training, and post-surgical rehabilitation
• Collaborate with nursing and rehab staff to coordinate care and optimize outcomes
• Document patient progress in the EMR and adjust care plans for continuous improvement
• Coach patients and families toward lasting mobility, safety, and independence

Qualifications

• Current California Physical Therapy License (required)
• Compassion and experience working with geriatric and rehab populations
• Strong communication, documentation, and teamwork skills
• SNF or inpatient rehab experience preferred
• Available for a 13-week full-time contract assignment

Knowledge & Skills

• Skilled in evaluations, goal-setting, and evidence-based treatment planning
• Proficient with manual therapy techniques, balance training, and post-op rehab
• Comfortable using EMR systems for accurate documentation and progress tracking
• Ability to collaborate effectively within interdisciplinary teams
• Patient-centered coaching and education for seniors and families
