Roles & Responsibilities

• Perform comprehensive evaluations and design individualized, evidence-based treatment plans
• Deliver one-on-one therapy using manual techniques, balance training, and post-surgical rehab
• Collaborate closely with nursing and rehab colleagues to coordinate patient-centered care
• Document patient progress accurately in the EMR and adjust plans to maximize outcomes
• Coach patients and families to improve mobility, safety, and independence

Qualifications

• Current California Physical Therapy License required
• Genuine compassion and experience working with geriatric and rehabilitation populations
• Strong communication, documentation, and teamwork skills
• SNF or inpatient rehab experience preferred
• Available for a full-time, 13-week contract assignment

Knowledge & Skills

• Skilled in manual therapy techniques
• Experience with balance training and fall prevention strategies
• Post-surgical rehabilitation knowledge
• Comfortable using EMR systems for documentation and care coordination
• Patient-centered communication and effective teamwork
• Accurate clinical documentation and progress tracking
