Roles & Responsibilities

• Perform thorough evaluations and design individualized, evidence-based treatment plans that help seniors regain mobility
• Apply manual therapy, balance training, and post-surgical rehabilitation techniques to improve patient outcomes
• Collaborate with nursing and rehab staff to coordinate care and achieve best-in-class results
• Document patient progress and update plans of care in the EMR for continuity and quality tracking
• Coach patients and families toward lasting mobility, safety, and independence through education and encouragement

Qualifications

• Current California Physical Therapy License is required
• Compassion and experience working with geriatric and rehabilitation populations
• Strong communication, documentation, and teamwork skills
• SNF or inpatient rehab experience preferred
• Ability to work full-time for a 13-week contract assignment

Knowledge & Skills

• Skilled in physical therapy assessment and treatment planning
• Proficient with manual therapy techniques, balance training, and post-surgical rehab
• Comfortable using EMR systems for documentation and tracking progress
• Excellent interpersonal skills for coaching patients and collaborating with interdisciplinary teams
• Detail-oriented approach to documentation and clinical outcomes
