Roles & Responsibilities

• Evaluate patients and design individualized treatment plans that restore strength, mobility, and independence
• Deliver targeted physical therapy interventions to improve mobility, balance, and functional outcomes
• Maintain accurate and timely documentation of patient progress to support clinical decision-making
• Collaborate with physicians, nurses, and rehab professionals to ensure coordinated, patient-centered care
• Educate patients and families on exercise programs and recovery techniques to extend gains beyond sessions

Qualifications

• Active California Physical Therapy license or eligibility to obtain one
• Graduate of an accredited Physical Therapy program
• Prior SNF and/or acute care experience preferred
• Strong communication and teamwork skills to thrive in a multidisciplinary setting

Education

• Graduate of an accredited Physical Therapy program

Knowledge & Skills

• Skilled in patient evaluation and designing individualized treatment plans
• Experience delivering therapeutic interventions for mobility, balance, and functional improvement
• Comfortable with clinical documentation and progress tracking
• Excellent collaboration and communication with care teams and families
• Patient education and motivational coaching to support recovery
