Physical Therapist - SNF

Roles & Responsibilities
- Provide skilled physical therapy evaluations and develop individualized treatment plans for SNF residents
- Deliver rehabilitative care focused on restoring functional mobility, independence, and quality of life
- Document clinical findings and progress notes accurately and in a timely manner
- Collaborate with nursing, OT, SLP and the interdisciplinary team to coordinate resident care
- Lead therapeutic exercise, gait training, balance and transfer training sessions
- Engage residents in meaningful activities and support a positive, person-centered environment
Qualifications
- Valid PT License in California (required)
- BLS certification (required)
- 1+ year of SNF or rehabilitation experience preferred; new grads with strong clinical spark are welcome
- Comfort working with geriatric populations and ability to collaborate across disciplines
- Strong documentation, communication, and time management skills
Knowledge & Skills
- Therapeutic exercise, gait training, balance and transfer training expertise
- Experience with SNF/long-term care rehab protocols and documentation practices
- Ability to create home programs and plan safe discharges
- Excellent interpersonal skills for working with residents, families, and the care team
