Roles & Responsibilities

• Provide compassionate bereavement counseling and support to patients and their families
• Conduct home visits across the service area to assess needs and deliver services
• Develop and facilitate bereavement support groups and educational sessions
• Collaborate with the hospice interdisciplinary team to coordinate care and resources
• Assess, document, and track family and patient needs, interventions, and outcomes
• Connect families with community resources, volunteers, and ongoing supports
• Deliver culturally sensitive care and tailor interventions to individual family situations
• Participate in team meetings, case reviews, and quality improvement initiatives

Qualifications

• Bachelor's degree in social services, counseling, or psychology (required)
• Master's degree in a related field preferred
• Healthcare Provider Level BLS required
• Experience or education in grief and loss counseling required
• Recent hospice experience preferred; additional medical setting experience a plus
• Valid driver's license, use of personal automobile, and required auto liability/property damage insurance
• Ability to perform home visits and maintain reliable transportation
• Strong communication, documentation, and community resource coordination skills

Education

• Bachelor's degree in social services, counseling, or psychology (required)
• Master's degree in social work, counseling, psychology, or related field (preferred)

Knowledge & Skills

• Grief counseling techniques and bereavement support best practices
• Understanding of hospice care philosophy and community-based service delivery
• Comfort and effectiveness conducting home visits and working in varied settings
• Excellent interpersonal skills for family-centered support and interdisciplinary collaboration
• Documentation, assessment, and outcome tracking in clinical settings
• Cultural sensitivity and ability to adapt support to diverse populations
• Ability to coordinate volunteers and community resources to enhance care
