Roles & Responsibilities

• Provide compassionate, hands-on care to residents, assisting with activities of daily living
• Monitor and document resident condition, reporting changes promptly to the nursing team
• Perform vital signs, basic assessments, and timely, accurate charting
• Support mobility and safe transfers to reduce fall risk and maintain dignity
• Assist with meals, hygiene, and comfort measures while promoting independence
• Collaborate with interdisciplinary team members to deliver individualized care plans
• Follow infection control and safety protocols to protect residents and staff
• Maintain professional communication with residents, families, and colleagues

Qualifications

• Current LNA (Licensed Nursing Assistant) certification for VT or eligible to obtain VT/LNA credentials
• Reliable personal transportation (own vehicle required)
• Must have own housing arrangements prior to assignment start
• Willingness and ability to work both weekends when needed
• Valid professional background check and immunizations as required by facility
• Strong interpersonal skills and a positive, patient-centered attitude

Education

• Completion of an approved Nursing Assistant program or equivalent training
• High school diploma or GED preferred

Knowledge & Skills

• Skilled in assisting with ADLs (bathing, dressing, toileting, feeding)
• Proficient in taking and recording vital signs and observing resident status
• Basic documentation and electronic charting familiarity
• Good time management and ability to prioritize in busy environments
• Knowledge of infection control, safety, and resident rights
• Team-oriented communication and ability to follow care plans
