Roles & Responsibilities
- Provide compassionate, hands-on care to residents, assisting with activities of daily living
- Monitor and document resident condition, reporting changes promptly to the nursing team
- Perform vital signs, basic assessments, and timely, accurate charting
- Support mobility and safe transfers to reduce fall risk and maintain dignity
- Assist with meals, hygiene, and comfort measures while promoting independence
- Collaborate with interdisciplinary team members to deliver individualized care plans
- Follow infection control and safety protocols to protect residents and staff
- Maintain professional communication with residents, families, and colleagues
Qualifications
- Current LNA (Licensed Nursing Assistant) certification for VT or eligible to obtain VT/LNA credentials
- Reliable personal transportation (own vehicle required)
- Must have own housing arrangements prior to assignment start
- Willingness and ability to work both weekends when needed
- Valid professional background check and immunizations as required by facility
- Strong interpersonal skills and a positive, patient-centered attitude
Education
- Completion of an approved Nursing Assistant program or equivalent training
- High school diploma or GED preferred
Knowledge & Skills
- Skilled in assisting with ADLs (bathing, dressing, toileting, feeding)
- Proficient in taking and recording vital signs and observing resident status
- Basic documentation and electronic charting familiarity
- Good time management and ability to prioritize in busy environments
- Knowledge of infection control, safety, and resident rights
- Team-oriented communication and ability to follow care plans
