Roles & Responsibilities
- Assess patient function and develop individualized occupational therapy plans that promote independence
- Provide hands-on treatment, adaptive equipment recommendations, and skilled interventions in the SNF setting
- Document progress clearly and timely in patient records while tracking outcomes
- Collaborate with nurses, PTs, SLPs, and care teams to coordinate patient-centered rehab plans
- Educate patients and families on strategies for safe transfer, daily living activities, and home setup
- Monitor caseloads to ensure consistent, high-quality care and identify opportunities for long-term improvement
- Participate in team meetings and contribute to a supportive, growth-focused rehab culture
Qualifications
- Licensed Occupational Therapist (OTR/L) in California or eligible to obtain licensure
- Degree in Occupational Therapy (BS/OT or MS/OT/OTD) from an accredited program
- Experience in skilled nursing facility, geriatric rehab, or post-acute care preferred
- Current CPR/BLS certification (or willingness to obtain)
- Strong clinical reasoning, documentation skills, and ability to manage a consistent caseload
- Excellent communication and teamwork skills — you’ll be working closely with interdisciplinary teams
Education
- Bachelor's, Master's, or Doctorate in Occupational Therapy (required)
Knowledge & Skills
- Skilled in assessment and intervention planning for ADLs, transfers, and functional mobility
- Knowledge of adaptive equipment, splinting basics, and home safety recommendations
- Comfortable with outcome-driven documentation and discharge planning
- Ability to collaborate effectively with PTs, SLPs, nursing, and case management
- Strong patient- and family-education skills with a compassionate approach
- Time management and caseload prioritization in a SNF environment
- Familiarity with common SNF patient diagnoses and rehab progression
