Roles & Responsibilities

• Provide compassionate direct nursing care to long-term care residents, prioritizing safety and dignity.
• Administer medications and treatments accurately and document per facility policy.
• Perform resident assessments and support individualized care plans.
• Complete required MDS-related documentation as needed and collaborate on care planning.
• Communicate clearly with residents, families, and the interdisciplinary team to coordinate care.
• Maintain accurate clinical records and ensure regulatory compliance during every shift.

Qualifications

• Current, unrestricted Colorado LPN license (required).
• LPN credential and active clinical skillset in long-term care (LTC experience required).
• MDS experience preferred — brings you to the top of the list.
• Ability to work day shifts (6:00 AM - 6:30 PM) and support resident care needs with a positive attitude.

Education

• Graduate of an accredited Practical Nursing (LPN) program or equivalent.

Knowledge & Skills

• Strong understanding of long-term care practices and resident-centered care.
• Familiarity with MDS processes and documentation expectations.
• Medication administration and clinical assessment skills.
• Clear communication, teamwork, and time management in a fast-paced LTC setting.
• Commitment to resident safety, infection control, and regulatory requirements.
