Roles & Responsibilities

• Conduct thorough patient assessments and identify high-risk discharge planning needs
• Lead patient and family conferences to develop clear, safe discharge plans
• Coordinate care across settings including SNFs, home health, hospice, and LTACH partners
• Collaborate with physicians and interdisciplinary teams to ensure smooth transitions of care
• Document case management activities accurately in EMR systems (Meditech, CarePort)
• Arrange medications, transportation, and post-discharge resources to support patient safety
• Round on assigned patients to identify barriers to discharge and escalate as needed

Qualifications

• Current Registered Nurse (RN) license (able to practice in Colorado)
• Minimum 2 years of nursing experience (case management experience strongly preferred)
• Demonstrated knowledge of discharge planning, patient assessment, and patient/family conferences
• Experience coordinating care with SNFs, home health, hospice, and LTACH providers
• Comfortable using EMR systems; Meditech and CarePort experience is a plus
• Strong communication skills and ability to work well in a multidisciplinary team
