Roles & Responsibilities

• Provide compassionate, hands-on nursing care to long-term care residents during 12-hour day shifts
• Administer medications and treatments accurately and document according to facility protocols
• Perform resident assessments and proactively communicate changes in condition to the care team
• Collaborate with nurses, therapists, and support staff to create and follow individualized care plans
• Support activities of daily living and promote resident dignity, independence, and quality of life
• Maintain accurate clinical documentation and ensure compliance with facility and regulatory standards

Qualifications

• Active LPN license (New York preferred or eligible) — your ticket to practice and make an immediate impact
• Experience in long-term care (LTC) settings is highly valued
• Comfort and ability to work 12-hour day shifts consistently
• Strong clinical judgment, communication, and documentation skills
• CPR/BLS certification preferred or willingness to obtain

Education

• Practical Nursing diploma or completion of an accredited LPN program

Knowledge & Skills

• Medication administration and safe documentation practices
• Resident assessment and recognizing changes in condition
• Wound care basics and infection control awareness
• Electronic charting and timely clinical documentation
• Teamwork, time management, and patient-centered communication
