Roles & Responsibilities

• Provide compassionate, direct patient care and support daily needs
• Administer medications and treatments safely and accurately
• Monitor and document patient vital signs and clinical changes
• Collaborate with RNs and the care team to coordinate effective care
• Maintain clear, timely charting in the patient record
• Assist with admissions, discharges, and patient education to improve outcomes
• Promote patient safety and follow infection control and facility policies

Qualifications

• Valid Licensed Practical Nurse (LPN) credential required
• Current BLS/CPR certification (or willingness to obtain)
• Ability to pass background check and drug screening as required
• Comfortable working in a fast-paced clinical environment and collaborating with team members
• Prior experience in a similar care setting is a plus

Education

• Graduate of an accredited Practical/Vocational Nursing program (LPN diploma or equivalent)

Knowledge & Skills

• Clinical assessment and patient observation skills
• Medication administration and basic wound care
• Strong documentation and electronic health record (EHR) familiarity
• Clear communication and teamwork with interdisciplinary staff
• Time management and prioritization in busy shifts
• Knowledge of HIPAA and patient privacy practices
