Roles & Responsibilities

Provide compassionate, patient-centered bedside care consistent with LPN scope of practice
Administer medications and treatments accurately and safely
Monitor and document patient conditions, reporting changes to the nursing team
Collaborate with RNs, physicians, and support staff to deliver coordinated care
Perform skilled nursing tasks such as wound care, vital signs, and ADL assistance
Educate patients and families on care plans and discharge instructions
Contribute to quality improvement efforts and maintain accurate clinical documentation

Qualifications

Active LPN license in Pennsylvania (required)
Completion of an accredited practical nursing program or equivalent
Current BLS/CPR certification or willingness to obtain promptly
Clear background check and ability to meet facility onboarding requirements
Strong communication skills, reliability, and a collaborative spirit

Education

Diploma or certificate from an accredited practical nursing program (LPN)

Knowledge & Skills

Skilled in medication administration and basic clinical procedures
Comfortable with patient assessment, vital signs, and documentation
Familiarity with electronic medical records and charting best practices
Ability to prioritize tasks in a fast-paced healthcare environment
Team player mindset with strong resident/patient advocacy skills
