Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
Administer medications and treatments accurately and safely
Monitor and document vital signs, patient status, and changes in condition
Collaborate closely with RNs, CNAs, and the interdisciplinary team to deliver excellent care
Assist residents with activities of daily living while preserving dignity and independence
Maintain accurate clinical documentation and help ensure regulatory compliance

Qualifications

Valid LPN license in Pennsylvania (required)
Current BLS/CPR certification (required)
Prior experience in long-term care or skilled nursing preferred
Strong communication skills and a team-oriented attitude
Ability to pass background checks and meet facility immunization requirements

Education

LPN diploma or Practical Nursing certificate
High school diploma or GED

Knowledge & Skills

Medication administration and basic pharmacology
Wound care and basic dressing changes
Monitoring and interpreting vital signs
Comfortable with electronic health records and clinical documentation
Resident-centered care and good interpersonal skills
