Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients with professionalism and warmth
Administer medications and document care accurately and timely
Monitor patient conditions and communicate changes to the care team promptly
Support activities of daily living, promoting dignity and independence
Collaborate with nurses, therapists, and support staff to deliver coordinated care
Maintain accurate clinical records and follow facility policies and procedures

Qualifications

Active LPN/LVN license in Pennsylvania (or compact license where applicable)
Current BLS/CPR certification (or willingness to obtain promptly)
Comfortable with medication administration, basic wound care, and clinical documentation
Strong communication skills and a collaborative attitude
Ability to pass required background checks and health screenings

Education

Graduate of an accredited Practical Nursing program (or equivalent)
Completion of state-required nursing licensure for LPN/LVN

Knowledge & Skills

Medication administration and basic clinical assessment
Charting and electronic medical record (EMR) familiarity
Infection control practices and safe patient handling
Time management and prioritization during busy 12-hour shifts
Patient advocacy and effective communication with families and care teams
