Roles & Responsibilities

Provide compassionate, patient-centered LPN nursing care with professionalism and warmth
Administer medications and treatments accurately and document care clearly
Monitor patient status, report changes promptly, and collaborate with the care team
Assist with daily living needs while promoting patient dignity and independence
Support admission, transfer, and discharge processes with thorough documentation
Contribute to a safe, clean environment and follow facility policies and infection control practices

Qualifications

Current LPN license in Pennsylvania or eligibility to obtain licensure
Graduate of an accredited practical nursing program
Basic Life Support (BLS/CPR) certification or willingness to obtain
Strong communication skills and a team-focused attitude
Ability to pass required background checks, drug screens, and facility onboarding

Education

Graduate of an accredited LPN/Vocational Nursing program
High school diploma or equivalent

Knowledge & Skills

Medication administration and accurate charting
Clinical assessment and basic patient monitoring
Familiarity with electronic health record documentation
Good time management, prioritization, and patient advocacy skills
Comfort working as part of an interdisciplinary care team
