Contract Type: Per-Diem

Shift Type: 8-hour shifts; exact days and start/end times to be confirmed with facility (schedule to be determined)

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct resident care including assistance with activities of daily living
Administer medications and treatments accurately and document in the medical record
Monitor and record vital signs, reporting changes in condition to the RN or provider
Collaborate with the care team to develop and follow individualized care plans
Support a safe, clean, and therapeutic environment for residents
Communicate clearly and respectfully with residents, families, and team members to ensure continuity of care

Qualifications

Current LPN license in Pennsylvania (required)
Completion of an accredited practical nursing program
Current CPR/BLS certification or willingness to obtain upon hire
Strong communication skills and a compassionate bedside manner
Ability to pass facility-required background checks and health screenings

Education

Graduated from an accredited practical nursing (LPN) program
High school diploma or GED

Knowledge & Skills

Medication administration and safe documentation practices
Basic wound care and infection control procedures
Ability to take and interpret vital signs and recognize changes
Familiarity with electronic health record charting or willingness to learn
Teamwork, time management, and resident-centered care focus
