Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in accordance with care plans
• Administer medications and treatments accurately and document all care thoroughly
• Monitor patient status and report changes to the care team in a timely manner
• Assist with activities of daily living including bathing, grooming, and mobility support
• Collaborate with nurses, therapists, and facility staff to deliver coordinated care
• Maintain a safe and clean environment while following infection control and safety protocols
• Engage residents with dignity and respect to support their emotional and social needs

Qualifications

• Current LPN license in the state of Pennsylvania or compact equivalency where applicable
• Active CPR/BLS certification preferred or willingness to obtain upon hire
• Previous experience in long term care, skilled nursing, or similar clinical setting is highly valued
• Strong communication and interpersonal skills to work effectively with residents and team members
• Reliable, compassionate, and dedicated to delivering high quality patient-centered care

Education

• LPN diploma or certificate from an accredited nursing program
• High school diploma or GED required

Knowledge & Skills

• Proficient in medication administration and basic wound care
• Familiarity with care plan documentation and electronic health records
• Good assessment skills and ability to recognize changes in patient condition
• Strong time management and prioritization during shift work
• Teamwork mindset with the ability to collaborate across disciplines
• Commitment to resident safety, infection control, and regulatory compliance
