Roles & Responsibilities

• Evaluate patient function and develop individualized treatment plans that drive real progress
• Deliver hands-on and therapeutic interventions to restore mobility and independence
• Document patient status and progress clearly and promptly in the medical record
• Collaborate with interdisciplinary team members to coordinate outstanding patient care
• Educate patients and families on home exercise programs and safe activity progression
• Measure outcomes and adjust plans to ensure measurable, meaningful gains

Qualifications

• Current, unrestricted Illinois Physical Therapist license (required)
• Basic Life Support (BLS) certification or willingness to obtain prior to start
• Comfort working independently in a fast-paced clinical environment
• Strong communication skills and a patient-centered approach
• Travel flexibility and ability to complete credentialing in a timely manner

Education

• Doctor of Physical Therapy (DPT) preferred; equivalent PT degrees considered

Knowledge & Skills

• Skilled in patient assessment, manual therapy, and therapeutic exercise prescription
• Experience with gait training, balance and vestibular rehabilitation
• Familiarity with common modalities and adaptive equipment
• Proficient documentation skills and comfort with electronic medical records
• Ability to set clear goals and track functional outcomes
