Roles & Responsibilities

• Provide compassionate, resident-centered nursing care consistent with the care plan
• Administer medications and treatments safely and accurately
• Perform head-to-toe assessments and report changes in condition promptly
• Assist with activities of daily living while promoting resident dignity and independence
• Document care and resident status in the electronic medical record in a timely manner
• Collaborate with RN staff, therapists, and interdisciplinary team members to coordinate care
• Support infection control practices and maintain a safe environment for residents

Qualifications

• Current and active LPN license in Pennsylvania
• Completion of an accredited practical nursing program or equivalent
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Ability to pass background checks and required immunizations
• Strong communication skills and a team-oriented mindset
• Previous long-term care or skilled nursing experience preferred

Education

• Vocational Nursing Diploma or equivalent required
• Active LPN license in PA required

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and infection control procedures
• Resident assessment and care planning fundamentals
• Familiarity with electronic medical records and clinical documentation
• Good interpersonal skills and the ability to work collaboratively
• Time management and prioritization in a fast-paced care setting
