Roles & Responsibilities

Provide compassionate, direct patient care consistent with facility policies
Administer medications and treatments safely and accurately
Monitor, record and report patient vital signs and condition changes
Collaborate with nurses, physicians and interdisciplinary team members to deliver coordinated care
Educate patients and families on care plans, medications and follow-up needs
Maintain accurate documentation in the patient record and follow established care plans
Adhere to infection control practices and safety protocols to protect patients and staff

Qualifications

Current, active LPN license in Pennsylvania (required)
Current BLS/CPR certification (or ability to obtain upon hire)
Preferred: prior experience as an LPN in a long-term care or skilled nursing setting
Ability to pass background check and drug screening
Strong communication skills and a patient-centered approach

Education

LPN diploma or certificate from an accredited program
High school diploma or GED

Knowledge & Skills

Solid clinical assessment and basic nursing skills
Medication administration and documentation
Familiarity with electronic health record systems
Infection prevention and control practices
Time management, organization, and teamwork
