Roles & Responsibilities
- Conduct comprehensive occupational therapy evaluations and assessments to identify baseline function and goals
- Develop and implement individualized treatment plans that promote independence and meaningful progress
- Provide skilled OT interventions focused on activities of daily living, functional mobility, cognition, safety, and adaptive strategies
- Collaborate closely with physical therapists, speech therapists, nursing staff, physicians, and case managers for coordinated care
- Educate patients and caregivers on treatment plans, home safety, and proper use of adaptive equipment
- Maintain accurate, timely documentation in compliance with facility policies and regulatory requirements
- Participate in interdisciplinary team meetings and care planning sessions to advocate for patient needs
- Uphold professional, ethical, and state practice standards in all clinical activities
Qualifications
- Current Occupational Therapist license in the state of California
- Graduate of an accredited Occupational Therapy program
- Experience in a Skilled Nursing Facility or post-acute care setting preferred; new graduates are encouraged to apply
- Strong clinical reasoning and patient-centered care skills
- Excellent communication, documentation, and organizational abilities
- Ability to work independently as well as part of a multidisciplinary team
Education
- Graduate of an accredited Occupational Therapy program
Knowledge & Skills
- Skilled in patient assessment and individualized treatment planning
- Expertise in ADLs, functional mobility, cognition, and safety-focused interventions
- Familiarity with adaptive equipment and home safety recommendations
- Strong documentation practices and regulatory compliance awareness
- Effective interdisciplinary collaboration and care coordination
- Clear communicator with caregivers and clinical teams
- Good organizational skills and time management in a clinical setting
