Roles & Responsibilities

• Conduct comprehensive occupational therapy evaluations and assessments to identify baseline function and goals
• Develop and implement individualized treatment plans that promote independence and meaningful progress
• Provide skilled OT interventions focused on activities of daily living, functional mobility, cognition, safety, and adaptive strategies
• Collaborate closely with physical therapists, speech therapists, nursing staff, physicians, and case managers for coordinated care
• Educate patients and caregivers on treatment plans, home safety, and proper use of adaptive equipment
• Maintain accurate, timely documentation in compliance with facility policies and regulatory requirements
• Participate in interdisciplinary team meetings and care planning sessions to advocate for patient needs
• Uphold professional, ethical, and state practice standards in all clinical activities

Qualifications

• Current Occupational Therapist license in the state of California
• Graduate of an accredited Occupational Therapy program
• Experience in a Skilled Nursing Facility or post-acute care setting preferred; new graduates are encouraged to apply
• Strong clinical reasoning and patient-centered care skills
• Excellent communication, documentation, and organizational abilities
• Ability to work independently as well as part of a multidisciplinary team

Education

• Graduate of an accredited Occupational Therapy program

Knowledge & Skills

• Skilled in patient assessment and individualized treatment planning
• Expertise in ADLs, functional mobility, cognition, and safety-focused interventions
• Familiarity with adaptive equipment and home safety recommendations
• Strong documentation practices and regulatory compliance awareness
• Effective interdisciplinary collaboration and care coordination
• Clear communicator with caregivers and clinical teams
• Good organizational skills and time management in a clinical setting
