Roles & Responsibilities
- Deliver compassionate basic nursing and restorative care within facility policies to improve residents’ daily lives
- Attend shift report and promptly communicate significant resident observations to licensed staff
- Assist residents with personal hygiene, bathing, grooming, and dressing according to individualized care plans
- Support ambulation, range of motion, transfers and positioning to maintain mobility and safety
- Provide feeding, hydration, toileting, continence care, and monitor input/output as directed
- React to emergency situations per facility policy to secure a safe environment for residents and staff
- Follow infection control techniques and maintain resident confidentiality at all times
- Document resident care accurately and maintain up-to-date Resident Care Plan knowledge
- Encourage resident participation in care to promote independence and quality of life
- Support nursing team by reporting supply/equipment needs and performance concerns
Qualifications
- Active New Hampshire LNA license (must be in-hand at time of submission)
- Minimum 1+ year of recent experience as an LNA (required)
- BLS (AHA) certification current (required)
- COVID-19 vaccination required (declinations/exemptions not accepted)
- Must have own transportation (facility is in a very rural area; no public transit)
- Willingness to float to units where experienced (required)
- Two supervisory references from the last two years of employment (required for profile)
- Certifications and license must be active at time of submission
- Previous travel experience and rural/small-facility experience are a plus
- Locals residing within 50 miles of the facility will not be accepted
Knowledge & Skills
- Resident care and Activities of Daily Living (ADLs)
- Patient transfers, turning, positioning and safe ambulation techniques
- Infection control practices and resident safety protocols
- Feeding, hydration assistance, toileting, continence care, and I&O monitoring
- Range of motion exercises and restorative nursing support
- Accurate documentation and familiarity with PointClickCare EMR
- Ability to follow Resident Care Plans and communicate with licensed staff
- Emergency response, fall prevention and de-escalation strategies
- Respect for resident rights, confidentiality, and person-centered care
