Roles & Responsibilities

• Perform comprehensive physical therapy evaluations and assessments to understand each patient’s needs
• Develop and implement individualized treatment plans that promote mobility, strength, balance, and functional independence
• Provide skilled interventions including gait training and therapeutic exercise to restore function
• Monitor patient progress and modify treatment plans to achieve meaningful outcomes
• Collaborate with occupational therapy, speech therapy, nursing, physicians, and case managers to deliver coordinated care
• Educate patients and caregivers on exercises, safety strategies, and home programs to support long-term success
• Maintain accurate, timely documentation in compliance with facility policies and regulatory standards
• Participate actively in interdisciplinary team meetings and care planning discussions
• Adhere to state practice acts, professional guidelines, and ethical standards while championing patient-centered care

Qualifications

• Current Physical Therapist (PT) license in the state of California
• Graduate of an accredited Physical Therapy program
• Experience in a Skilled Nursing Facility or post-acute care setting preferred; new graduates are encouraged to apply
• Strong clinical decision-making and patient care skills
• Excellent communication, documentation, and time-management abilities
• Ability to work independently and collaboratively within a multidisciplinary team

Education

• Graduate of an accredited Physical Therapy program

• Expertise in physical therapy techniques for mobility, balance, gait, and strength
• Skilled patient evaluation and individualized treatment planning
• Strong patient care focus with an emphasis on functional outcomes
• Clear and compassionate communication with patients, families, and team members
• Efficient documentation and familiarity with regulatory requirements
• Clinical decision-making in a post-acute or SNF setting
• Time management to balance caseload and documentation
