Roles & Responsibilities
- Perform comprehensive physical therapy evaluations and assessments to understand each patient’s needs
- Develop and implement individualized treatment plans that promote mobility, strength, balance, and functional independence
- Provide skilled interventions including gait training and therapeutic exercise to restore function
- Monitor patient progress and modify treatment plans to achieve meaningful outcomes
- Collaborate with occupational therapy, speech therapy, nursing, physicians, and case managers to deliver coordinated care
- Educate patients and caregivers on exercises, safety strategies, and home programs to support long-term success
- Maintain accurate, timely documentation in compliance with facility policies and regulatory standards
- Participate actively in interdisciplinary team meetings and care planning discussions
- Adhere to state practice acts, professional guidelines, and ethical standards while championing patient-centered care
Qualifications
- Current Physical Therapist (PT) license in the state of California
- Graduate of an accredited Physical Therapy program
- Experience in a Skilled Nursing Facility or post-acute care setting preferred; new graduates are encouraged to apply
- Strong clinical decision-making and patient care skills
- Excellent communication, documentation, and time-management abilities
- Ability to work independently and collaboratively within a multidisciplinary team
Education
- Graduate of an accredited Physical Therapy program

- Expertise in physical therapy techniques for mobility, balance, gait, and strength
- Skilled patient evaluation and individualized treatment planning
- Strong patient care focus with an emphasis on functional outcomes
- Clear and compassionate communication with patients, families, and team members
- Efficient documentation and familiarity with regulatory requirements
- Clinical decision-making in a post-acute or SNF setting
- Time management to balance caseload and documentation
- Interdisciplinary collaboration for coordinated care planning
