Roles & Responsibilities
- Implement physical therapy treatment plans developed by the supervising Physical Therapist to help patients regain mobility and function
- Provide skilled therapeutic interventions focusing on mobility, gait training, strength, balance, and functional activities
- Assist patients with therapeutic exercises and functional mobility tasks to promote independence
- Monitor patient response to treatment and communicate progress or concerns to the supervising PT
- Complete accurate and timely documentation of treatment sessions and patient progress
- Collaborate with occupational therapists, speech therapists, nursing staff, and other care team members to deliver coordinated care
- Educate patients and caregivers on exercises, safety techniques, and mobility strategies to support home readiness
- Follow facility policies, state regulations, and professional standards to maintain a safe, compliant practice environment
Qualifications
- Current Physical Therapist Assistant (PTA) license in the state of California
- Graduate of an accredited Physical Therapist Assistant program
- Experience in a Skilled Nursing Facility or post-acute care setting preferred
- Strong patient care and clinical skills with a focus on geriatric populations
- Effective communication and time-management abilities
- Ability to work independently while collaborating within a multidisciplinary team
Education
- Graduate of an accredited Physical Therapist Assistant program
Knowledge & Skills
- Solid knowledge of mobility, gait training, strength and balance interventions
- Skilled in safe patient handling and transfer techniques
- Proficient in clinical documentation and progress note accuracy
- Ability to assess patient response and adjust care communications to the supervising PT
- Experience collaborating with interdisciplinary teams and coordinating care
- Patient education skills to teach exercises, safety practices, and functional strategies
