Roles & Responsibilities

• Evaluate residents' functional mobility and design individualized treatment plans
• Deliver evidence-based therapeutic interventions focused on restoring independence
• Document patient progress clearly and timely in the facility electronic record
• Collaborate with nurses, OT, SLP and interdisciplinary team to coordinate care
• Educate residents and families on home programs, safety and fall prevention
• Support transitions of care and contribute to quality improvement efforts in the SNF

Qualifications

• Doctor of Physical Therapy (DPT) or degree in Physical Therapy
• Open to all experience levels, including new graduates
• State PT license required or ability to obtain Indiana licensure prior to start
• Comfort working in a skilled nursing facility (SNF) practice setting
• Strong communication skills and ability to work well within an interdisciplinary team

Education

• Doctor of Physical Therapy (DPT) preferred; equivalent PT degree accepted

Knowledge & Skills

• Skilled in patient assessment, treatment planning and functional mobility training
• Knowledge of geriatric rehab principles and SNF clinical workflows
• Experience with documentation and outcome tracking in a clinical setting
• Ability to apply therapeutic modalities and adaptive equipment safely
• Strong clinical reasoning, patient education and family engagement skills
