Roles & Responsibilities
- Evaluate residents' functional mobility and design individualized treatment plans
- Deliver evidence-based therapeutic interventions focused on restoring independence
- Document patient progress clearly and timely in the facility electronic record
- Collaborate with nurses, OT, SLP and interdisciplinary team to coordinate care
- Educate residents and families on home programs, safety and fall prevention
- Support transitions of care and contribute to quality improvement efforts in the SNF
Qualifications
- Doctor of Physical Therapy (DPT) or degree in Physical Therapy
- Open to all experience levels, including new graduates
- State PT license required or ability to obtain Indiana licensure prior to start
- Comfort working in a skilled nursing facility (SNF) practice setting
- Strong communication skills and ability to work well within an interdisciplinary team
Education
- Doctor of Physical Therapy (DPT) preferred; equivalent PT degree accepted
Knowledge & Skills
- Skilled in patient assessment, treatment planning and functional mobility training
- Knowledge of geriatric rehab principles and SNF clinical workflows
- Experience with documentation and outcome tracking in a clinical setting
- Ability to apply therapeutic modalities and adaptive equipment safely
- Strong clinical reasoning, patient education and family engagement skills
