Roles & Responsibilities

• Evaluate residents to develop individualized, evidence-based treatment plans
• Deliver skilled therapeutic interventions focused on mobility, balance, and functional independence
• Collaborate with nurses, OT, SLP and the care team to coordinate care and optimize outcomes
• Document clear, timely progress notes and update care plans to reflect resident goals
• Coach residents and families on home programs, safety, and fall prevention
• Participate in care conferences and discharge planning to ensure smooth transitions

Qualifications

• PT or DPT credential (PT/DPT listed as required skills)
• Open to all experience levels — any experience accepted, including new graduates
• Comfort working in a Skilled Nursing Facility (SNF) practice setting
• Available for day shifts; role specifies no floating to other units

Education

• Doctor of Physical Therapy (DPT) or equivalent professional PT degree — new grads encouraged to apply

Knowledge & Skills

• Strong clinical skills in therapeutic exercise, transfers, gait training and balance interventions
• Familiarity with SNF documentation and regulatory expectations
• Excellent communication and teamwork with interdisciplinary staff and families
• Ability to prioritize caseloads and manage time effectively
• Compassionate, resident-centered approach focused on functional outcomes
