Roles & Responsibilities
- Assess patients’ functional status and create individualized treatment plans
- Observe and evaluate treatment effects and recommend changes to the care team
- Perform appropriate assessments and implement therapeutic procedures
- Provide rehabilitative care for patients with neuromuscular, skeletal, integumentary, pulmonary and cardiovascular disorders
- Document patient progress and communicate findings with physicians and staff
- Educate patients and families on home programs, safety and prevention strategies
- Collaborate with an interdisciplinary team to coordinate high-quality patient care
- Contribute to a supportive, patient-centered environment in a rural hospital setting
Qualifications
- Current Colorado Physical Therapy license (required)
- Basic Life Support (AHA) certification (required)
- Willingness to relocate to and work in a rural community
- Strong communication, documentation and teamwork skills
- Commitment to patient-centered care and ongoing professional growth
Education
- Doctor of Physical Therapy (DPT) or equivalent required for PT licensure; graduation from an accredited physical therapy program
Knowledge & Skills
- Proficient in patient assessment and treatment planning
- Skilled at evaluating treatment effects and adjusting care accordingly
- Able to select and implement therapeutic procedures to restore function
- Clinical knowledge of neuromuscular, skeletal, integumentary, pulmonary and cardiovascular conditions
- Strong patient education, documentation and interdisciplinary collaboration skills
- Comfortable working in a community-focused, resource-aware environment
