Roles & Responsibilities

• Assess patients’ functional status and create individualized treatment plans
• Observe and evaluate treatment effects and recommend changes to the care team
• Perform appropriate assessments and implement therapeutic procedures
• Provide rehabilitative care for patients with neuromuscular, skeletal, integumentary, pulmonary and cardiovascular disorders
• Document patient progress and communicate findings with physicians and staff
• Educate patients and families on home programs, safety and prevention strategies
• Collaborate with an interdisciplinary team to coordinate high-quality patient care
• Contribute to a supportive, patient-centered environment in a rural hospital setting

Qualifications

• Current Colorado Physical Therapy license (required)
• Basic Life Support (AHA) certification (required)
• Willingness to relocate to and work in a rural community
• Strong communication, documentation and teamwork skills
• Commitment to patient-centered care and ongoing professional growth

Education

• Doctor of Physical Therapy (DPT) or equivalent required for PT licensure; graduation from an accredited physical therapy program

Knowledge & Skills

• Proficient in patient assessment and treatment planning
• Skilled at evaluating treatment effects and adjusting care accordingly
• Able to select and implement therapeutic procedures to restore function
• Clinical knowledge of neuromuscular, skeletal, integumentary, pulmonary and cardiovascular conditions
• Strong patient education, documentation and interdisciplinary collaboration skills
• Comfortable working in a community-focused, resource-aware environment
