Roles & Responsibilities

• Provide compassionate hands-on nursing care to residents, ensuring dignity and comfort
• Perform routine assessments and communicate changes in condition to the care team
• Administer medications and treatments safely and accurately according to orders
• Document care and progress in the patient's chart clearly and timely
• Collaborate with interdisciplinary team members to create and follow individualized care plans
• Support admission, discharge, and transfer processes with professionalism and empathy
• Educate residents and families on care plans and wellness strategies in a respectful manner

Qualifications

• Must hold an active LPN license in Massachusetts (required)
• Current CPR/BLS certification (preferred or obtain upon hire)
• Experience in long-term care, rehab, or skilled nursing preferred
• Strong communication skills and the ability to work well within a multidisciplinary team
• Demonstrated ability to manage time, prioritize tasks, and maintain accurate documentation

Education

• Completion of an accredited LPN program (diploma or certificate)
• High school diploma or equivalent

Knowledge & Skills

• Skilled in medication administration and wound care fundamentals
• Proficient with basic nursing assessments and vital signs monitoring
• Comfortable using electronic medical records and standard clinical documentation
• Knowledge of infection control practices and resident safety protocols
• Ability to provide compassionate, person-centered care in a fast-paced setting
