Roles & Responsibilities

• Provide compassionate, hands-on care to residents — assisting with ADLs (bathing, dressing, feeding) with dignity and respect
• Observe and report changes in patient condition to the nursing team promptly
• Assist with safe transfers and mobility, promoting independence whenever possible
• Document care accurately and efficiently to support continuity and compliance
• Collaborate with interdisciplinary team members to deliver person-centered care
• Maintain a clean, safe environment and follow infection control procedures

Qualifications

• Current LNA / Licensed Nursing Assistant certification (able to practice in VT; applicable NH credentials noted in job title)
• Prior experience in long-term care or rehab settings preferred but caring attitude and willingness to learn are essential
• Reliable transportation required — this assignment expects you to get to work on time, rain or shine
• Ability to work the 3:00 PM - 11:00 PM shift and every-other-weekend rotation
• Strong communication skills and a team-first mindset

Education

• Completion of a state-approved nursing assistant program or equivalent training

Knowledge & Skills

• Skilled in basic clinical care and ADLs with a focus on patient dignity
• Comfortable with documentation and following care plans
• Knowledge of infection prevention practices and safety protocols
• Good interpersonal skills for working with residents, families, and the care team
• Adaptability in a fast-paced rehab/long-term care environment
