Roles & Responsibilities

• Provide compassionate, skilled nursing care to patients in their homes
• Complete thorough OASIS assessments and ensure accurate, timely documentation
• Develop and implement individualized care plans in partnership with patients and caregivers
• Coordinate care across the interdisciplinary team to keep patients safe and progressing
• Monitor clinical status, intervene as needed, and escalate changes promptly
• Educate patients and families on disease management, medications, and preventive care
• Advocate for patient needs and ensure compliance with home health standards and policies

Qualifications

• Active Registered Nurse (RN) license required
• OASIS experience required and comfortable performing comprehensive assessments
• Home health clinical experience and case management experience required
• Travel nurse experience strongly preferred
• Ability to commit to the assignment with no requested time off during the contract

Education

• Active RN license (required)
• BSN preferred but not required

Knowledge & Skills

• Skilled in OASIS assessment and home health documentation
• Strong clinical judgment and independent decision-making in home settings
• Excellent communication and care coordination skills
• Time management and ability to prioritize a caseload
• Comfortable working autonomously and engaging patients and families
