Roles & Responsibilities

• Evaluate posture, gait, balance, coordination and strength to build individualized treatment plans
• Develop and implement therapy programs that restore mobility and promote independence
• Lead, guide and direct the delivery of physical therapy services in compliance with local, state and federal regulations
• Collaborate with nursing, rehab staff and families to ensure cohesive care plans
• Document progress, update care plans and communicate outcomes to the interdisciplinary team
• Educate residents and caregivers on safe mobility techniques and home exercise programs
• Ensure skilled nursing/long-term care compliance and quality standards are consistently met

Qualifications

• Valid, unrestricted Physical Therapy license in the state of Missouri (or Compact allowed per job posting)
• Minimum 1 year of Physical Therapy experience (required)
• BLS (Basic Life Support) certification (required)
• Valid driver’s license in the state of employment
• LTC or rehab experience (preferred)
• Comfort leading therapy services and providing oversight in a long-term care setting

Education

• Meets educational requirements for state licensure (e.g., accredited Physical Therapy degree such as DPT or equivalent required for licensure)

Knowledge & Skills

• Strong patient assessment skills including gait analysis, balance and coordination evaluation
• Skilled in treatment planning and strength assessment to restore function
• Knowledgeable of skilled nursing/long-term care regulations, procedures and best practices
• Ability to lead service delivery and mentor therapy staff
• Good communication and documentation skills for interdisciplinary collaboration
