Roles & Responsibilities
- Perform comprehensive medical assessments to understand each participant’s full clinical picture
- Develop and implement individualized care plans that support independence and dignity
- Evaluate and treat acute illnesses, intervening promptly to prevent unnecessary hospital stays
- Participate actively in discharge planning to ensure smooth transitions across care settings
- Conduct home visits as clinically indicated to deliver person-centered care in the community
- Coordinate care across the full continuum of PACE services to keep care seamless and connected
- Assist in developing and reviewing clinical policies and procedures to strengthen quality and safety
- Participate in the provider on-call rotation to support participants and the care team when needed
- Collaborate with interdisciplinary team members to deliver holistic, team-based care
Qualifications
- Active Nurse Practitioner licensure in Massachusetts or eligibility to obtain licensure
- National NP certification (appropriate to your specialty) and current clinical credentials
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) preferred
- Experience with geriatrics, chronic disease management, home-based care, or PACE programs preferred
- Current CPR/BLS certification (or willingness to obtain) and ability to participate in on-call rotation
- Valid driver’s license and reliable transportation for home visits
Education
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) preferred
- NP national certification required
Knowledge & Skills
- Clinical assessment and acute illness evaluation with strong diagnostic reasoning
- Individualized care planning and coordination across multiple service lines
- Experience in home-based care and conducting safe, effective home visits
- Discharge planning and transitions-of-care best practices
- Collaboration with interdisciplinary teams and effective communication with patients and families
- Familiarity with PACE program goals and senior-focused, patient-centered care
- Ability to contribute to clinical policy and procedure development
