Shift Type: Monday-Friday, 8:00 AM - 5:00 PM (Day); occasional weekend coverage and emergency on-call as needed

Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Perform thorough OASIS assessments and create individualized care plans
- Deliver skilled nursing care during patient home visits with compassion and professionalism
- Document patient encounters accurately in the HealthWyse/Careficient EMR
- Coordinate care with interdisciplinary teams and community providers
- Educate patients and caregivers on condition management, medications, and safety
- Manage and prioritize a travel caseload, adjusting schedules as needed
- Respond to emergency on-call situations promptly and appropriately
- Maintain compliance with facility policies, background checks, and safety protocols
Qualifications
- Current Registered Nurse (RN) licensure (must meet state requirements)
- OASIS experience is required — you know assessments and outcomes
- Home Health and Hospice experience required
- BLS certification (required) and current
- Familiarity with HealthWyse / Careficient EMR systems
- Valid driver’s license in good standing and ability to travel between patient homes
- Strong clinical assessment, documentation, and time-management skills
- Comfortable adapting to schedule changes and working independently in the community
Knowledge & Skills
- Outcome and Assessment Information Set (OASIS) expertise
- Home health nursing workflows and hospice care principles
- Proficient charting in HealthWyse and/or Careficient EMR
- Skilled in patient education, care coordination, and discharge planning
- Excellent communication, clinical judgment, and triage skills
- Safe driving and navigation for patient-to-patient travel
