Roles & Responsibilities

• Provide skilled physical therapy visits in patients’ homes across a rural territory
• Perform thorough assessments and develop individualized care plans
• Complete accurate OASIS documentation and timely charting to support quality care
• Coordinate with nursing, OT, ST, social work and hospice teams for holistic care
• Educate patients and caregivers to promote independence and safe function at home
• Drive safely in winter conditions and manage a geographically large caseload
• Maintain current clinical skills and participate in required compliance documentation

Qualifications

• Active Physical Therapist license for New Hampshire (Compact allowed) — license verification required
• Minimum 2+ years of current Physical Therapy experience in Home Health
• Home Health and/or Hospice experience — required
• OASIS documentation experience — required
• Active American Heart Association BLS (AHA) — required (documented)
• Permanent address must be included in submission; NO LOCALS — must live 50+ miles from facility
• Supervisory signed reference from the past year of employment — required
• Current Physical Therapy skills checklist within past year — required
• Reliable personal vehicle and comfort driving in winter weather

Education

• Graduate of an accredited Physical Therapy program (DPT or equivalent) preferred
• Active licensure as a Physical Therapist in New Hampshire (required)

Knowledge & Skills

• Skilled in OASIS documentation and home health clinical documentation
• Experience across age groups: pediatric through geriatric care
• Understanding of hospice care principles and palliative approaches
• Strong patient education, fall prevention and functional mobility training
• Care coordination and interdisciplinary communication
• Time management and ability to manage a rural caseload independently
• Safe winter driving and navigation of remote routes
