Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in the rehab center
• Administer medications and treatments accurately and document per facility policy
• Perform thorough assessments and monitor changes in patient condition
• Collaborate with therapists, physicians, and care team members to coordinate care plans
• Educate residents and families on care plans, recovery goals, and safe discharge practices
• Maintain accurate clinical documentation and ensure compliance with regulations
• Promote safety, infection control, and a healing environment for all residents

Qualifications

• Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license — RN preferred
• Current BLS/CPR certification required
• Experience or interest in post-acute/rehabilitation nursing is a strong plus
• Comfortable working overnight shifts and collaborating in an interdisciplinary team
• Ability to maintain clinical documentation and follow facility protocols

Education

• Graduate of an accredited nursing program (LPN diploma or ADN/BSN for RN)

Knowledge & Skills

• Skilled in patient assessment and care planning in a rehab/post-acute setting
• Proficient with medication administration and safety practices
• Strong clinical documentation and charting skills
• Good communication and teamwork — building rapport with patients and families
• Basic wound care, restorative nursing, and mobility support knowledge preferred
