Roles & Responsibilities
- Provide compassionate, skilled direct patient care to assigned residents
- Administer medications and treatments accurately and document per facility policy
- Monitor patient status, communicate changes promptly to the care team, and escalate concerns
- Perform thorough and timely charting in the electronic health record
- Collaborate with nurses, therapists, and providers to support individualized care plans
- Support admissions, discharges, and transfers as needed with professionalism
- Contribute to a positive unit culture by mentoring others and practicing excellent communication
Qualifications
- Current LPN license (practical nurse) in the state of assignment or eligible to obtain one
- BLS/CPR certification required (or willingness to obtain prior to start)
- Must be able to commit to weekend coverage as noted in the assignment
- Reliable transportation required
- Ability to pass background checks, health screenings, and facility onboarding requirements
- Strong teamwork, time management, and patient-focused attitude
Education
- Graduate of an accredited Practical Nursing (LPN/LVN) program; LPN certificate or diploma
Knowledge & Skills
- Medication administration and documentation best practices
- Basic assessment skills and ability to recognize clinical changes
- Familiarity with electronic health records and accurate charting
- Infection control and standard precautions
- Clear verbal and written communication with patients, families, and care teams
- Ability to prioritize tasks in a fast-paced long-term/post-acute setting
