Roles & Responsibilities

• Provide compassionate, skilled direct patient care to assigned residents
• Administer medications and treatments accurately and document per facility policy
• Monitor patient status, communicate changes promptly to the care team, and escalate concerns
• Perform thorough and timely charting in the electronic health record
• Collaborate with nurses, therapists, and providers to support individualized care plans
• Support admissions, discharges, and transfers as needed with professionalism
• Contribute to a positive unit culture by mentoring others and practicing excellent communication

Qualifications

• Current LPN license (practical nurse) in the state of assignment or eligible to obtain one
• BLS/CPR certification required (or willingness to obtain prior to start)
• Must be able to commit to weekend coverage as noted in the assignment
• Reliable transportation required
• Ability to pass background checks, health screenings, and facility onboarding requirements
• Strong teamwork, time management, and patient-focused attitude

Education

• Graduate of an accredited Practical Nursing (LPN/LVN) program; LPN certificate or diploma

Knowledge & Skills

• Medication administration and documentation best practices
• Basic assessment skills and ability to recognize clinical changes
• Familiarity with electronic health records and accurate charting
• Infection control and standard precautions
• Clear verbal and written communication with patients, families, and care teams
• Ability to prioritize tasks in a fast-paced long-term/post-acute setting
