Roles & Responsibilities

• Provide compassionate, patient-centered nursing care for residents at Bloomfield Nursing and Rehab
• Conduct thorough assessments and develop individualized care plans
• Administer medications and treatments safely and accurately
• Monitor patient status and communicate changes to the interdisciplinary team
• Document care clearly and promptly in the medical record
• Collaborate with therapists, physicians, and support staff to optimize outcomes
• Educate patients and families on care plans, discharge instructions, and follow-up needs
• Support a positive, respectful environment that promotes dignity and recovery

Qualifications

• Current Registered Nurse license in New Mexico or eligible for licensure
• Basic Life Support (BLS) certification required
• Advanced certifications such as ACLS or PALS are a plus
• Prior experience in long-term care, rehab, or skilled nursing preferred
• Strong clinical assessment, medication administration, and documentation skills
• Reliable, professional, and a team player who puts patients first

Education

• Associate Degree in Nursing (ADN) required, BSN preferred

Knowledge & Skills

• Skilled in patient assessment and care planning
• Proficient with medication administration and IV therapy basics
• Solid documentation practices and charting accuracy
• Able to prioritize, multitask, and respond quickly to changing situations
• Excellent communication and patient/family education abilities
• Team-oriented with experience in interdisciplinary collaboration
