Roles & Responsibilities
- Provide compassionate, high-quality nursing care to residents in Bloomfield Nursing and Rehab
- Administer medications and treatments accurately and safely
- Perform focused assessments and promptly communicate changes in condition to the care team
- Document patient care thoroughly and maintain accurate medical records
- Assist with wound care, ADLs, and mobility support to promote resident independence
- Collaborate with interdisciplinary team members to develop and follow individualized care plans
- Support a positive, resident-centered environment that respects dignity and comfort
Qualifications
- Active Licensed Practical Nurse (LPN) license in New Mexico or eligible for licensure
- Current CPR/BLS certification
- Previous LPN experience in long-term care, skilled nursing, or rehab settings preferred
- Strong clinical assessment skills and ability to prioritize in a fast-paced environment
- Excellent communication and teamwork skills
Education
- Completion of an accredited Practical Nursing program or equivalent
- High school diploma or GED required
Knowledge & Skills
- Medication administration and documentation best practices
- Basic wound care and infection control procedures
- Familiarity with electronic health records and accurate charting
- Understanding of resident rights, geriatric care principles, and safety protocols
- Ability to triage, escalate concerns, and work collaboratively with nursing staff
