Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a rehab and long-term care setting
- Administer medications and treatments safely and accurately, following facility policy
- Monitor and document patient conditions, reporting changes promptly to the care team
- Assist with activities of daily living and mobility while promoting resident dignity
- Collaborate with nurses, therapists and physicians to execute individualized care plans
- Support transitions of care, ensuring clear communication and accurate documentation
- Maintain infection control standards and a safe, clean care environment
Qualifications
- Active LPN license in Massachusetts (required)
- Graduate of an accredited practical nursing program or equivalent vocational certificate
- Current BLS/CPR certification (preferred or to be obtained upon hire)
- Comfortable working in a rehab/long-term care environment; experience in similar settings a plus
- Strong communication, time management, and teamwork skills
Education
- Diploma or certificate from an accredited practical nursing program (LPN)
Knowledge & Skills
- Medication administration and documentation best practices
- Basic patient assessment and vital signs monitoring
- Wound care, catheter care, and ADL assistance fundamentals
- Electronic medical record/charting familiarity (facility-specific training provided)
- Infection control, safety protocols, and regulatory compliance
- Effective communication with residents, families, and interdisciplinary teams
