Roles & Responsibilities

• Provide thorough patient evaluations across pediatric to geriatric populations and translate findings into clear treatment plans
• Deliver hands-on and evidence-based outpatient physical therapy interventions for orthopedic, neurological, and general medical caseloads
• Document all patient care accurately and efficiently in McKesson EMR
• Collaborate with providers, nursing, and ancillary team members to coordinate exceptional patient-centered care
• Monitor patient progress, revise plans as needed, and communicate outcomes to patients and families to support meaningful recovery
• Maintain compliance with facility policies, safety protocols, and regulatory requirements
• Mentor and support colleagues when appropriate to foster a strong, skilled therapy team

Qualifications

• Active New Hampshire state physical therapy license (Compact allowed)
• Minimum 2 years of current Physical Therapy experience — NO NEW GRADS
• Outpatient experience required; inpatient experience highly preferred
• Current American Heart Association BLS certification (document required)
• Proficiency with McKesson EMR
• Supervisory signed reference from past year (required) and completed physical therapy skills checklist
• License verification and all current licenses/certifications in hand at time of submission
• Permanent address must be included and must be 50+ miles from the facility (NO LOCALS)

Knowledge & Skills

• Patient evaluations and treatment planning for adolescent through geriatric patients
• Orthopedics and neurological rehabilitation expertise
• Experience managing a general medical outpatient caseload
• Familiarity with McKesson EMR for documentation and workflows
• Strong clinical reasoning, patient education, and interdisciplinary communication skills
• Ability to manage higher patient ratios (typically 12-15 patients per shift) while maintaining quality care
